When submitting your entry, please send a screenshot of the entire song downloaded on iTunes along with your submission.
Please note that many people mistake this screen for the Apple Music screen or the song playback screen.

*This application is to be submitted via Google Form. If you are prompted to log in, please login with your Google account and enter your information.
After entering the required information, please click the "Submit" button at the bottom of the form, and you will see the message "Response Recorded. Please note that you will not receive an application completion e-mail.

Personal information you provide will be used only for this campaign. We will destroy the information in an unrecoverable form as soon as the campaign is over.
*Please note that failure to complete the application form by the deadline will invalidate the application.
*If we deem that an entry is fraudulent, such as multiple consecutive entries by the same person under multiple accounts, we may invalidate the entry without prior notice.
Please note that we are unable to confirm or change any information after the application has been completed.
Once the information entered in the application form has been submitted, we will not be able to make any changes to the information. Even if you submit your information more than once, only the first submission will be considered valid.
*If we cannot confirm that the purchase has been made on iTunes, or if the entry is incomplete, you will not be eligible for the gift.
